Enable mod_extforward in lighttpd properly

Question

I am trying to configure lighty so it can work with CloudFlare using mod_extforward to show the IP from a header in access.log.

The lighttpd.conf looks like this:

I had accesslog enabled via "lighty-enable-mod accesslog", so disabled it and enabled via the config file, adding mod_accesslog just before the mod_extforward. With this new config, mod_accesslog seems to not log anything, with the old config, I am getting the nginx server IP (because mod_extforward loads before mod_accesslog, I think).

Anyone know what's wrong?
