Do you have device-mapper multipath set up? ... Also kernel messages might be helpful to diagnose the problem. –  Dmitri Chubarov Sep 14 '12 at 5:30

device-mapper multipath is not set up, two physical NICs have been binded into one... checking kernel messages... thanks. –  Vladimir Sep 14 '12 at 7:09

if there are relevant messages in dmesg output, could you please post them. I always mount iscsi targets via multipath mountpoints even if there exists a unique path. This guarantees at least protection from scsi device name changes. Could you run multipath, mount the nodes created under /dev/mapper instead of /dev/sdX nodes and check if the problem persists? –  Dmitri Chubarov Sep 14 '12 at 16:25
