I am trying to add a couple of RAID 5 arrays to my second adapter. Adapter 0 is two disks in a raid 1 which is running the OS. This seems to be working fine. Attempting to add the second array gives this failure:

I'm at a bit of a loss what to check next. I do have some other servers that are (supposed) to be configured the same way on the same hardware that is working. Only the OS drives on this server are listed in /dev/sda# . Any ideas are greatly appreciated, please and thank you.
