@jbx Disk performance is tricky; there is sequential and random; sequential is when all data is written/read from the disk in large contiguous block. Random is when it's not (databases for example). When you use fio you'll only be correlating performance assuming that's what went wrong originally. You're better off running sar -d 1 and watching %util and await columns to determine if your disks indeed are overloaded. You could use iotop to see what is writing where; but that still doesn't tell you if the disks are overloaded (and if it's RAID which disk may be failing).

@jbx you'll see load; but you won't see a lot. The reason being the file that was read was probably already read and therefore already in the kernel cache; so unless the file changes (or the caches are dropped) the kernel isn't going to read it again. If you're using PHP and have an opcode cache the same logic applies.
