From the spamc man page: "Note that there is a very slight chance mail will be lost here, because if the fork-and-exec fails there's no place to put the mail message". You can avoid this problem by having Exim launch spamc instead; this is done with the transport_filter option on your transport. You may also want to use the --no-safe-fallback, so that Exim defers delivery, keeping the message on the queue for later, in the event that spamd can't process the message.

In fact I found this bug while searching: <bugs.exim.org/show_bug.cgi?id=321>;. Shame their robots.txt file prevents search engines from indexing the bugs. :(

That did the trick! The only disadvantage is that now my LMTP server is exposed to any process on the system. Fortunately iptables can help here: iptables -A OUTPUT --out-interface lo --protocol tcp --dport 24 --match owner ! --uid-owner Debian-exim --jump REJECT --reject-with icmp-admin-prohibited prevents ant other processes from interfering with the LMTP service on port 23.
