MaxRequestsPerChild specifies the maximum number of requests that a single worker can process before being reaped and respawned. These are not concurrent requests.. The setting that specifies how many workers can handle requests concurrently is MaxRequestWorkers. So yes, if set to 200, the process will be killed and respawned after processing 200 requests. It won't just 'die out'.

Read (r) has nothing to do with accessing a directory. It's the execute (x) bit that allows you to cd into a directory. What happened to you doesn't happen to me though....

it will display all log-ins from up to the date the /var/log/wtmp file got rotated. You can check the other wtmp files in /var/log with the -f switch.
