comment CentOS Server possibly hacked
@David regardless to file name... I'm sure there are ways that this whole issue can be handled, in your case it lays through knowledgeable SA, I suggest you explore that option.

@David bar3648.js is just a file name and it looks like file name was generated name anyway, so it could be pretty much anything.

it's nearly impossible to guide you through how to resolve this as it doesn't seem like you have even basic understanding, I'd recommend you to reach out to professional to take some necessary actions.

comment EC2 Single Instance Returning Port 80 for SSL
ssl.conf that comes w/ mod_ssl will provide you ssl functionality out of the box (without redirecting anywhere), if you're on port 80 that's not secure, it should be port 443 or just https instead of http.

@Michael as I mention before; I used that just for test to show it to you, so you do NOT need to install it, basically it acts as a http client (a.k.a. browser).

nagios-plugins is just for a test, so you can see output of phpinfo();.

comment I don't have Sendmail but port 25 is still using by Sendmail
@Kami, I just updated my answer with sendmail as well, as it seems like you're running sendmail and not postfix.

if it's already set to all from localhost than I'm guessing you didn't restart your postfix yet, as that's the only configuration change is needed in order to start listening on all interfaces.

you might want to include relevant apache configuration file, i just tried in my own environment and SERVER_PORT returned 443.

comment MySQL not accepting password, responds with using password: NO
@parthianshot sorry to disappoint you, but I didn't look at bytecode to see it(. I just copy & paste it to my terminal, removed -h and hit enter, my answer was right there as it threats that part as name for database therefore no -p.

@NathanC thanks for trying) I doubt that it's postfix though.

@NathanC sendmail_path is already pointing to that (see my updated question).

comment emails are coming from localhost instead of php sendmail_from
@NathanC echo(); shows me right sendmail_from (XXX@XXX.TLD) as what was set in ini_set earlier, yet email still showing up as apache@localhost.

comment emails are coming from localhost instead of php sendmail_from
@NathanC I'm using my own environment, disabling of ini_set() has been DEPRECATED as of PHP 5.3.0 and REMOVED as of PHP 5.4.0. (I'm using 5.3.3).

comment RHEL servers with no access to Internet constantly polling DNS for xmlrpc.rhn.redhat.com
could be 'cause you chkconfig service without actually stopping it and now it's not appearing as it didn't start after reboot.
