I see what you mean. Is there any reason why your web server cannot have the single-label names bound to the relevant sites? You could, for example, bind the host header "www" to the same site as "www.mydomain.com"

If the clients are Windows machines, they may also be using NetBIOS broadcast name resolution or LLMNR which won't hit the DNS server, and may also give different results from nslookup

It doesn't explicitly mention it, but I would imagine that it's not possible to upgrade from the 32-bit version of Server 2008.

That strikes me as less secure than the default. Are you sure you want the WWW publishing service to run under a different context, or could you perhaps be needing to run an Application Pool under a different context (for connections to remote servers and so on)?

I took a look this morning before disabling RRAS - the wizard applies a set of pretty restrictive IP filters (both inbound and outbound) that would definitely revent the machine refreshing its policy.
