What I'd do to narrow down the problem: What happens if you remove the script altogether? What happens if you use /bin/true instead of the script? Could it be a simple permissions problem?

This really doesn't make a lot of sense. If you want the frontend server to act as a simple conduit, why don't you directly DDOS-protect your backend server and get rid of that frontend server?

Is this your real script? Because there's a bug, and it has nothing to do with the keepalived upgrade. You call ping_return, but the function is called ping_test.

SIGTERM means that some process is sending the TERM signal to apache. The same is also done by /etc/init.d/apache stop, so it doesn't necessarily mean something bad. You could start checking the logs in order to see who was online at that moment or what else could have been going on.
