Ah, I see; thanks. I dug a bit more into it, but when I found myself writing an Apache module solely for the purpose of calling ap_add_common_vars, I took a step back and decided I was better off just parsing X-Forwarded-For in Varnish.

Using the same clients (Chrome and httpie) to check. The setenvif_module is statically compiled in, as reported by httpd -t -D DUMP_MODULES.

Also, it's nice to be able to use as few Varnish instances as possible to keep hitrates up - if you have to put one Varnish instance on each app, you're spreading your cache out. And you should be using Varnish.

The errors are likely coming from one of two sources; either the application (phpMyAdmin) is deciding you aren't authorized when trying to perform actions, or the web server isn't allowed to access the files to serve the request at all. Figuring out which of those two is the problem is the first step. The latter is easier to fix, so try looking at that first: check the Apache error logs, and look at the permissions on the files you're requesting (via ls -l). They should have read permissions for all users, and the containing directories should be executable by everyone.

You might need to set error_reporting to E_ALL to see errors in the log.

Right. So, do you have a plan for how you're going to monitor httpd and do failover, or is that part of what you want to know? Sidenote: sessions are usually managed in your application, not within Apache.
