Can you add access_log and error_log directive in your server block and see what is going on through logs? your headers are fine.

Different linux distributions have different directory structures, and also different versions of ldap may have different directory structures. But i dont think it will make big difference, just use the file you already found.

I did something like this to detect ddos on ngix. I achieved it by parsing access log. I did a cron job to parse access log and count unique ip connections per hour.

Another slow first request happened when I had a big iptables list. So it is worth making sure if you are getting same latency in other services like ftp or ssh.

Usually usedns is the reason for the delay, but moduli (openssh.com/faq.html#3.3) can also contribute to the delay. did you restart the sshd server after making changes to ssh_config?

can you please go to postgresql directory and search for uninstall-postgresql.app? that will uninstall postgresql completely
