I don't know where the device file is. If I did I'd have done what you said. That is the problem, I want to mount a virtual disk on the host so I can store ISO images. I have nowhere else to put them.

I think you meant for PATH_TRANSLATED to be $document_root$fastcgi_script_name because now it seems to work!

Okay, I've done all that but $_SERVER["PHP_SELF"] is still /index.php/index.php - and PATH_TRANSLATED was already set the way you just mentioned.

I've updated and included the entire server config. It's just for cacti, and there are no rewrites as you can see. I'm not sure what you mean by routing system.

That is everything relevant to the problem. It is a bit of a mess because I've been trying a few different things. The result is the same without the include, because at this point it doesn't add anything. So between it and the include there is a lot of redundancy.
