comment nginx sending incomplete response
i can confirm again, this is not a firewall or client-side bug. it's caused by the server. one more fact, that we noticed. only chrome browsers seem to have issues. firefox never had this problem. with chrome, sometimes it's ending up with empty response; other times the same request is successful, without any issues. i will add further logging to the question description on top.

i tried with smartphone via mobile instead of local wifi, too, and got the same problem. also, access.log shows similar. first a 1size response, followed by another download request with "random" size, whenever i try to refresh.

download with wget/curl is OK. but Chrome and Safari browser on windows and unix fail for unknown reason. in access.log, we can see that also other visitors are facing the issue.

comment KVM: RAM Usage vs. actual consumption on VPS
@MichaelHampton: (The other "duplicate" thread is about another VZ type, doesn't help on this.) I see the same result, 300MB of memory in use. However, the hoster worries about this VPS consuming almost 100% of memory. That's the same as we see in free/top, almost all RAM full.

@CristianCiupitu: it's KVM - not a "software virtualization" but hardware-like.

comment max #process for nginx sync to php-fpm
though php-fpm config is set to log level=notice, it only tells about WARNING: [pool www] seems busy (you may need to increase pm.start_servers, or pm.min/max_spare_servers). it reached maximum, so i'd expect it to use a queue - not disconnect nginx.

comment max #process for nginx sync to php-fpm
The queue seems to fail or break. Whenever the request can't be served quickly enough, nginx error recv() failed (104: Connection reset by peer) while reading response header from upstream, client: and WARNING: The maximum number of processes has been reached. suggesting to consider max values. but that's not wanted. instead, i want it to wait, not to request even more at the same time or http deny with err code 502, also fail :-(

that's the conflict, isn't it? nginx will try to get even more php process' though the max number is limited and will never be more because of php-fpm's config. i wonder how this could be considered in order to have nginx not ending up in errors 502 if the number is exceeded.

comment Proxy Bypass Rule (serverside)
maybe i misconfigured the squid proxy, but when accessing whatismyip.com (via acl serverside setting through the proxy), it shows the IP of the proxy server. will "directdomains" really be connecting the client with the target destination or will squid always remain as man in the middle?
