comment How to improve availability on a starting up PHP+MySQL application?
You are trying to boil the ocean. Each one of availability, performance and scalability requires separate, in-depth investigation and an intimate knowledge of your application/environment. You're kind of cavalier about having MySQL replicating across data centers in particular. That is a hard problem to solve and not inexpensive. There are solutions for it like Percona's XtraDB Cluster (Galera) and Continuent's Tungsten Cluster but each requires a substantial investment in time and money. There are solutions but be realistic, you're not going to change this overnight and it's going to cost $$.

comment Unbelievable issue : a single apache process takes 4 GB of memory
@db_ch, each PHP codebase will contribute its own share to the memory size of an httpd/apache2 process.

Do you have several VirtualHosts each with its own PHP codebase?

@Poni, Well if you want more than port redirection then you probably need to run a reverse proxy...

comment Development Environment Sprawl
"Is anyone else seeing this Environment-Sprawl?" Hell, yes. But depending upon the business it may be necessary. We have branch & trunk enviros for Dev, Sales & QA and Staging and Production (branch of course) enviros. Our architecture also has a backend & frontend and the backend has additional enviros (Preview and What's running on Prod). It's madness!

You haven't even said what kind of application this is for. Note that MySQL Cluster is generally unsuitable for web applications.

comment How to involve QA with the Infrastructure Department
It sounds like the failure was in the provisioning of (backup) infrastructure. How is QA supposed to help with that? That's not their core competency. Perhaps you should partner with a Managed Services Provider (e.g. IaaS provider).

You can centralize your accounts in a directory server (LDAP). Maybe some fancy logic in MySQL Proxy can help you? Re: reporting, not sure. Reporting means different things to different people.
