Are you sure about this statement: "If there are any missing indexes (I don't think there are) they affect the old and new servers equally." I would start with the assumption that some of the indexes did not transfer with the restore, and take steps to make sure all of the indexes present in the source DB are actually present in the target DB too.

Ok, in that case, the vpn domain is taking precedent for some reason. I don't know if there's a way when you're logged into both to specify the precedent domain. Since you said you setup trusts, can you give your user in the vpn domain permission to sql server the same way your normal user has? That way you wouldn't have to use the runas /netonly command when vpn'd in.

I just had another thought. If you open SSMS and connect before you vpn in- I assume it doesn't stop working after you connect to the vpn?

Are the VPN and local network on the same subnets? (From a command prompt, do ipconfig /all when vpn'd in and see what your two IPs are.) If the networks are overlapping this might cause it. If this is the case, you can just change your local subnet to a different IP range.

Sorry I wasn't clear about how to use the /netonly switch. It must be used in conjunction with the runas command. I updated my answer with better instructions.
