had /etc/init.d/nginx been executabe, then you'd been able to run the service script without permission denied. Check ls -l /etc/init.d/nginx and you'll see that it's not.

take the php stuff out of the equation for now -- just monitor the mod rewrite logs and you're gonna find out where you fail for sure.

you need to enable rewrite logging to determine where you fail. Error 500 is not related to rewrite rules.

well, this example should redirect the m.example.com, so try enabling some logging httpd.apache.org/docs/current/mod/mod_rewrite.html and check your logs. It will say what the value of HTTP_HOST is while evaluating each request.

what about sar, sysstat, dmesg, memory etc. How much memory does squid consume while acting strangely? iostat, iotop, top? Any other weird things in log? does it swap?

Have you tried starting the script manually with /sbin/service script start. what says chkconfig --list script. You need to find out weather your script has triggerd but failed or if it has not run. you don't need sudo probably, since root starts the services on the server .
