It was indeed a switching loop. As soon as we removed the redundant cable (which was there for other reasons) the network went rock solid again and hasn't had a problem yet.

Upvoted because this is exactly what I've thought and where I've been looking at and tt was the most logical explanation. The problem was a different one though.

It's commodity hardware with custom parts. It has been up and running for 4+ years without problems on the same O/S installation and same software.

@ChrisS: Yes but if you server images from static.example.com then it'll also transmit all cookies of example.com without a reason. That's why you'll usually need a static-images-of-example.com (a completelly different domain. That's what stackoverflow, facebook etc do.
