We're no further along with understanding the cause of the problem, but we have confirmed that the Xserve RAID is the key component that's at fault; it's also connected (on the other controller) to another server, which has also seen these problems. We've chosen to address the problem by accelerating the retirement of that Xserve RAID, which was overdue anyway.

The filesystem is on an Apple Xserve RAID, which isn't displaying any problems (via the RAID Admin tool).
