My instance is a large (or xlarge, can't remember) one, so I don't think processing power is the issue here. One thing to note is that I am running sshd on a non-standard port (i.e. not 22), so it's probably given the lowest priority by some QoS somewhere.

There's definitely some throttling going on, I'm trying to figure out whether it's my ISP or Amazon. I don't see how changing ports would help me do that. It's still an interesting experiment, so I'll have a go at it.

RAM usage was fine. I was logged in right before the hang and was running htop and everything looked normal.

Apache (+PHP with APC) and MySQL. I did not check their logs, thinking one of them failing couldn't bring the entire system to a freeze.

No, there's no such path. But I don't see why it matters, since it should be internally rewritten to ask.php/$1, so there's no security issue :/
