Try to keep script and data separate. Even if an attacker manages to drop something in /uploads/, it shouldn't be executable. Security in layers is key.

Testing in a physically separate environment (virtual or otherwise) is a good idea. It's the safest way to realize that a simple volume backup/restore just won't work.

The documentation isn't bad. Pages 10-20 explain the core process that needs to be followed. Migrating a DC (especially SBS) is a delicate procedure; I strongly recommend taking the time to read the WHOLE document. It's so much easier than needing to recover from making a mistake.

No. Both system-state backup and full volume backup (ala Ghost) will include parts of the registry that are dependent on the computer hardware, and should only be restored to identical hardware.

I consider myself fairly proficient with procexp and autoruns. Last time I used these tools to remove a virus everything looked squeaky clean, but it turned out that the computer was still transmitting spam messages - enough to get our IP put on several SMTP blacklists. The only way to be sure of a clean box is to rebuild.

PHP runs in the same user context as the webserver, so it can create files and folders in any directory owned by the webserver. There are generally only a few folders like this (/uploads/ for example). I'm not sure about git/svn - could you add them to the group account that controls the website?
