@Redmumba, correct. The password would be exposed to a local attacker, or local malicious code with sufficient privileges. However, there are mitigating factors in our case to prevent escalation. Further, FTP passwords are already vulnerable to network interception.

@Stan, that means mysql isn't in the PATH. You can adjust that, or get the full mysql path from which then put it in the command: ssh server 'echo status|/full/path/to/mysql'

Yes, you're right that some browsers cache for performance (possibly longer than the TTL). However, you clearly can't rely on that for the correctness of your implementation. For one thing, it is trivial to disable Firefox's DNS cache manually or with an addon.

According to this MSDN SQL Server blog, 16 means, "the incoming user does not have permissions to log into the target database." That doesn't really explain why the error would fix itself without any changes, though.
