Nope. It is not secure. If the application has a flaw, then a remote user can write any php and then run any command as www-data.

If the IPs have the same 192.168.1.0/24 prefix does not always mean they are in the same VLAN. You can have PPP interfaces with /32 prefixes.

May be you have a hardware problem. Run a memtest86+. For this you will have to reboot the server. And the downtime will be a few good hours. Run other hardware tests... for CPU, etc... Check the temperatures on the server.
