It isn't likely that someone created a custom edit form - unless I am very much mistaken, there isn't anyone else here who would know how, even if they wanted to do it.

In Sharepoint Designer I can see that EditForm.aspx is the default edit document. When I open this up, it looks the same as EditForm.aspx in document libraries that don't have the problem - It shows [ Preview of List Form Web Part] and shows three columns that are replaced with the actual columns used in the document library when the page is delivered to the browser.

Name field is definitely still attached to the list - if I switch to Datasheet view then I can see it, and if I add one of the other Name field options to the view (Name, linked to document for example), I can edit it.

Unfortunately not - I only have one version of SharePoint and it is customised. You could always just ask the site administrator what version they use, although I think they would be suspicious (as am I) of why you want to know. Perhaps you can expand your question - people may sit on their hands if they think you are probing sites to discover potential to make mischief :-)

Unless it has been heavily customised it is usually pretty obvious from the UI, but perhaps you mean a specific version number, rather than WSS/MOSS 2003/7 or Sharepoint 2010 etc?

By the way, your choice of DNS provider may depend on exactly what you are using the domain for. WikiLeaks had their DNS record pulled last week by EveryDNS, who bought DynDNS. So if you are setting up a *Leaks website, you probably want to use someone else ;-)

Use labelling that can be removed, and remove it as soon as you remove a cable for a particular purpose. And never leave a cable connected at only one end for longer than absolutely necessary. Someone will plug it in where they shouldn't.

There are web parts that can access document libraries on other web applications and site collections - e.g. My Sharepoint Sites which displays documents I have recently edited on any site I specify. So this isn't technically impossible.
