I'm fairly sure you'd hit the parameter limit ARG_MAX after the shell expansion.. Better to write a small loop.

I stand corrected. You did post it. I've honestly never used mount to view mounts before, I usually use /etc/fstab and /proc/mounts, mount uses /etc/mtab.. I'm guessing you've rebooted and this still occurs?

Can't even execute strace.. useful. What about when logged in as another (non root) user, does it work then? Or is every non-root user failing to execute? this suggest checking shared libraries

ok, next I would try file /bin/echo and make sure it matches q/echo, then try strace q/echo ok and see what it is doing, or failing to do.

I'm seeing this also, it works for .php files, it works for static files that don't exist, but not for static files that DO exist.. odd indeed! I'm actually seeing "(null)" for static's that do exist. Link <http://server.org(null)> rel="self"

How many connections are there? Not sure if it would happen, but I don't know what it would do if you go over the max_connections.. perhaps you are using loads of pconnects? Hmm, also, check for open files and inodes.. and lots of things! gah, you sure it throws no errors? Perhaps you tell us how you are connecting then. What is the exact response you get?

Also, with no context, like the whole list, we can't see if you've allowed the traffic first and then attempted to rate limit, which would produce the same effect as not rate limiting at all..
