comment Limit the total number of emails per domain / per hour / per day
I have not used Hmail enough but from my experience, it was not possible. Maybe the documentation hides it somewhere, hmailserver.com/documentation/latest

Please add your full worker settings, this may help a lot to give an accurate answer.

comment apache's caching (mod_cache) vs squid reverse-proxy caching?
Yes, 3 machines is the bare minimum, 4+ is optimal. Server1 and Server2 would have Apache+MySQL+memcached. Server3 & 4 would have your load balancer (HAProxy) and static content cache (Varnish)

comment when to set up a mail server?
You dont need any MTA (Mail Transfer Agent) such as Postfix, sendmail, Qmail, etc. Php mail() function will handle the outgoing of emails. I am not aware of any requirement for mail() to work. You would install an MTA if you have local mailboxes.

You sadly have to upgrade if you need this feature.

Take the one you are more familiar with. By multiple instances of them I meant to have multiple http deamon instance running and load balancing requests.
