Your observation that /var/amavis/tmp goes missing from time to time might be related to your server using an SSD and you having disabled access times to be written. Then amavisd thinks it didn't actually need the temp directory and removes it. (Yes this behaviour is extremely dumb.)

Changing the default port from 22 to something arbitrary helps a lot to cut down on log spam by dumb scanning scripts. No added security, but I'd still recommend doing it.

You can use SpamAssassin with almost any mta. Definitely with postfix, qmail and exim. You definitely want a content (spam) filter with your mailsetup. Doesn't necessarily have to be SpamAssassin, but this one works fine. Remember that a good smtp setup will get rid of a lot of spam before it hits the expensive processing through a filter. PS: I'm a postfix person and like it very much, but I don't have a lot of experience with qmail or exim to be honest.

From my experience rsync is very likely to max out the IO performance on the Xserve, even with internal drives, and especially with FireWire devices.

rsync comes standard with Mac OS X and I expect it to be available on most Linux distros as well. You can just give it a try as rsync can use ssh as a transport.

Justin, you're actually right! It might still be a hosts.deny integration with the firewall that can produce this behaviour. (Though admittedly unlikely from the updated info.)
