For mac: dscacheutil -flushcache flushes the local cache in the operating system, not the bind name server. rndc flush will clear the bind name server on a mac which will affect all computers using that name server.

In my case, I was using a bitnami .ami and didn't realise that you need to log in as the user called bitnami, like: ssh -i <keyfile> bitname@<ec2-address>. Unfortunately the -v option didn't help me find this, but it's still very useful to check!

I had put an Alias directive in my config, eg Alias svn /repositories and then set up everything in <Location /svn/ >. The problem was exactly as in the FAQ link, although mysteriously, it worked perfectly for a month and then stopped on a Wednesday afternoon at tea time. If your repository is at "/" then you'll have an overlap with your DocumentRoot directive.
