Download the DTrace Toolkit (brendangregg.com/dtrace.html#DTraceToolkit). it contains a number of DTrace scripts relating to IO - iotop, iohog, iosnoop etc. That should get you going in the right direction.

If you put the FQDN in /etc/sysconfig/network then the 'hostname' and 'hostname -f' commands will return the same FQDN value. This is not really correct. 'hostname' should ideally simply return the actual hostname.

I wouldn't say a bogus timezone is a very good solution. It's a cheap hack and it may work for a while but you're gonna run into problems somewhere else.

Citrix allows you to disable cut and paste entirely. I find the "feature" quite annoying. SSH tunneling will not have any cut and paste monitoring, as you are actually working directly on the remote serer.

Yes you can use a single account on the bastion host for all the clients (perhaps using a ssh public key exchange) and then use different user accounts on the inside machine, if you want. The users will actually never see that they are logging into the bastion host at all. i.e. With the ProxyCommand specified for inside.machine in .ssh/config on the client side, every time the user does a "ssh userX@inside.machine", the ProxyCommand will automatically connect to bastion first and then use nc to proxy the connection through to inside.machine.

I've seen similar behaviour on a few Oracle Enterprise Linux boxes. You might want to double check on iproute2 - do a "ip rule show" and a "ip route show" and you might see something interesting.
