Hardware seems fine, and like i say, this happens across different physical hypervisors. It't not OOM killer as there is no OOM killer on SmartOS. Plus the QEMU instance is still running it does not exit. It's the guest instance that is rebooting.

can you show what an "apt-cache policy lsscsi" shows, as well as "cat /etc/debian_release". Also a pastbin of /var/log/dpkg.log may help show the history of package changes on the system helping to track down the cause.

Oh Also one cable failure should not effect access to either volume. There are two controllers in the MSA A and B, if either controller is lost (or the path to the controller) the other one will take over the vdisks it was responsible for. (Assuming your running multipath on Linux) try multipath -l from some informative output.

Ever find this update ? We just acquired another MSA2000sa and just hit the 16drive limit using the latest published firmware. Short of a new firmware we will be using LVM to stripe 2 volumes.

That looks to be the trick then. Firmware update should allow the connection to stay open. grub and login console are configured correctly the only issue is that iLO closes the connection.
