To clarify: Scheduled Tasks are run by making an HTTP call, so if there is no web server running, on-box tasks will fail.

The only thing that comes to mind is that something is periodically (or, worse, sporadically) overwriting the XML file where these are stored. Probably not helpful enough to be an answer, but maybe it gives you a place to start.

Most of these can be gotten pretty easily. There are 3rd party log analyzer tools for IIS logs, or you can write your own. I don't know anything about the administrator API, but if the CF logs aren't available through it, you can read them yourself. Depending on your SQL server variant, you can monitor what it is doing. There are tools for looking at Windows performance. I'd decide what you want, then figure out how to do it. There's always a way. The question comes down to whether it's worth the time to implement.

My list is far from canonical, but I think it's a pretty good start. Some of what you need to document will be dependent on your specific site and needs. Keep in mind that your requirements may change over time, and it's better to have documentation and not need it than the reverse.

Olaf might have meant something like memcached, but I think he was probably talking more about making sure that the db has sufficient RAM available to prevent disk thrashing (at least, that's what I'd look at for a first pass). If the db is having to swap too much, that's going to slow you down quite a bit.
