Good, so that would seem to indicate a DNS problem. If all your external testing hit the same Name Server(s) then you might try different Name server(s). In other words you might want to test in other external environments to rule out an overwhelmed Name server.

I am not sure what the default website has to do with it, but it appears to be a DNS problem if you are reliably able to access the website using the public IP address.

So when you access the website from the remote location via the internal IP address it works correctly? But if you use the public URL (IP or Domain name) it fails? Sounds like you might need a static route.

I would try something like Firebug or Fiddler and watch the requests as you try to load the page. Also, I suspect that if the DNS records are correct, the traffic is it at least making it to your edge router. You may want to dig your URL to verify DNS. You probably will want to post your configs in the question too. You want to rule out the firewall as well.
