Check how your host is implementing IP fail-over. If they can already guarantee that one and only one server will have the IP at a time, then you don't need to worry about split-brain scenarios. If you are doing this for yourself, you need a extra vote. This can come from another cluster node, a lightweight "witness" node, or a quorum disk. Depending on how the VPS environment is set up, a shared disk might not be practical.

Yes, the 1622 errors are logged in the event log. The COMPUTER$ account seems not to be able to access those locations, even though it is a domain member and every other domain member computer can access them (and so could this one until about a week ago... and the users don't have admin rights, so in theory can't break things. So much for that theory...)

You could also consider using CentOS clustering (a free version of Redhat Cluster Suite). The documentation can be a bit opaque, but once you have luci set up with fencing devices (easy on VMware) and a shared disk, it gets easier. Put the quorum partition on the shared disk, configure the rest of it as LVM, and use HA-LVM to prevent both nodes mounting it at once.

It prompts for a restart, and then gives the 1622 error for each package it tries to install when the system comes back up.
