I don't have a solution, but I wanted to express solidarity. I had this happen on one of my first Linux boxes in 2008 and it drove me insane, almost to the point of going back to Windows. I installed the next version of Ubuntu and the problem went away, never to return... which almost bothers me more than the original problem.

@AndrewB Ergh, I am. Now that I'm googling "PAM with SSH key" I'm seeing a hojillion folks with this problem. Hooray for asking the wrong questions. Thanks for the intel.

@PaulHaldane I'm seeing a lot of connections from imapd/pop3d but nothing that looks like connections from outside. Still looking.

@joeqwerty Google's surprisingly unhelpful - I did try the advice in this answer: serverfault.com/questions/525682/… but I couldn't get it to work.

The domain's MX entry points to Apps. I need mails sent to the domain from Exim are routed to Google instead of locally.

I appreciate that, and I'll likely be using it - but it's not exactly something I want to be running long-term. It's starting to appear as though the attacker guessed the subdomain to bypass CF - I changed it from the default, but only to a dictionary word. We'll see if it helps.
