I checked but there is no error. Also, I can start those services manually without problem. And nginx listen on port 80, Apache on 8080, OpenVZ and php-fpm all have their own ports.

Great comment. Not really. I do need to separate sites from LAN and from Internet (which I have port forwarding on router), I also use iptables to forward things line 127.0.1.2:80 to another host in a ssh tunnel. But all of them do not require to bind nginx to a specific IP as I thought. I already use different hostnames.

I added more detail. It indeed seems like IP assignation problem. I just don't know how it happens nor how to debug. nginx is on the last lines of the /var/log/boot.log. The IP is fixed and assigned by a DHCP server (which is a set-top-box).

I could modify your settings to match a pattern with regular expression. However the tricky part is there is no content-type header if it is accessed without extension, and nginx uses "Content-Type: application/octet-stream".

As the actual kernel in Debian Squeeze is 2.6.32, I think I'll have to get a new one from backports then try to increase cwnd to about 12 to fit my 16 KB file size.

@psusi There are/were multiple problems that I can only resume, not explain: 1/ APC cache miss (but OS cache hit for those PHP files, so very little disk I/O, less wait time but more avg I/O time, which mostly MySQL InnoDB transaction commit) 2/ APC cache hit but APC was using files (then OS cache miss, don't know why) 3/ brief, my question is "when cache worked badly, there is (almost) no disk I/O" - what you're saying is completely contrary to that.

@psusi I was nearly zero during two days, and I was on it, so I know that it was not down. The traffic was normal (a few dozens of thousand of visitors/day). The only difference in those two days is cache was full and heavily fragmented.

I've found the problem. See my answer below. About cookies, headers... I already ignore all them in the conf that I posted above. This is my set up for edge side caching of media content, so I don't care about IP address, cookie, user agent etc.

I abandonned the OpenVZ solution to install all services one physical server because I got a performance problem with OpenVZ. So I really don't need the answer for this question. However I give the bounty to SparX for the word "tricky". It's not only difficult for security but also for dimensionning.
